The Assuring Your Wishes registry is managed in a cooperative effort between your Local Agency and Assuringyourwishes.org in New City, NY.
- We are committed to respecting and protecting the privacy of everyone who files their advance directives with us.

- We will only provide access to your advance directives to those individuals to whom you have provided a passcode and your health care providers.
Other than your advance directives, none of your personal identifying information that is in our possession will be shared with anyone. People you authorize and your health care providers are the only ones who may view your advance directives. We do not share information with or sell it to third parties. We do not collect information from those who visit our website unless they have provided answers to a survey. We may collect statistics about your visit to our website, but you will not be identified in any summary of those statistics. Your authorization form will provide us with personal identifying information but this information will not appear on the website. This information will only be used to provide you with a unique identification number and to assure you and your health care providers that your advance directives can be located. You cannot submit your completed advance directives electronically. They must be mailed to your Local Agency. Our organization places a strong emphasis on confidentiality. Each staff member signs a confidentiality agreement with penalties associated with violating our policies. Each person associated with Assuringyourwishes.org has been thoroughly trained and they are the only ones who have access to your information. Their access to the website is password protected.
Documents are kept in a secure location. Once your documents have been scanned, they are returned to you with your wallet identification cards via US mail. We do not maintain any paper copies of your documents.
The server that hosts this website and stores your information is located in a locked facility. The website is backed up on a regular basis and is checked for viruses, and the firm that hosts the website has a disaster recovery system in place.
When your advance directives are scanned, they will be scanned to create a read-only PDF (portable document format) file that cannot be changed in any way. If you wish to make changes to your advance directives, you will need to complete new documents and send them to your Local Agency.
