Sharing Your Wishes™ Program Summary Report
This report begins with the background and rationale for the Sharing Your Wishes project and provides a description of the components of the three-year program design. In addition, the report provides a brief summary of each of the seven coalition's experience.
To create this tool kit of resources, each of the coalitions provided items developed as part of their three-year Sharing Your Wishes grant experience. The pieces included in this resource will be helpful not only to the current SYW coalitions, but also to other communities interested in improving their approach to advance care planning. Many are provided in easy-to-use formats so that you can adapt them for your community and purpose.

Terms of use. The items in this tool kit are part of the Sharing Your Wishes™ program. They are provided at no charge to communities wishing to improve their efforts to help people plan ahead for future health care decisions. Commercial use of any of these materials is not permitted. Many of the items are provided in easy-to-use formats so that you can adapt them for your community and purpose, such adaptation is permitted, however the use of the Sharing Your Wishes program name requires written permission by the Community Health Foundation of Western and Central New York.
